Each club is entitled to register an unlimited number of players on List B during the season. The list must be submitted by no later than midnight CET on the day before the match in question.

Is this really a fixed question? According to UEFA's rules:Each club is entitled to register an unlimited number of players on List B during the season. The list must be submitted by no later than midnight CET on the day before the match in question.
So there's no such thing as 'being included on a B List' - players are just arbitrarily added as and when the club sees fit.

Correct, according to Transfermkt Jordy joined Huddersfield from City in July 2015 for £183K. Apparently, they loaned him to Wigan for £171K then Walsall for £170K then to Bradford for £213K until the end of this season. How much City get out of those loan fees is anybody's guess but I reckon not much and probably zero.
One more to get. Here's a clue, he got booked against Wigan on Monday night.
