As for the match fixing, it's a very good question that I doubt will ever be answered without recorded evidence. If the FA is unable/unwilling to confront bad officiating because of a fear of calling the game into question.... theres no chance in hell that the possibility of match fixing would ever be entertained. Unfortunately, this will all be swept under the rug because the FA, clubs, and media are all on the gravy train and would never do anything to derail it. Just look at what has happened in Italy as an example.
If the FA/clubs were smart however, they would act proactively and have an inquiry/investigation/open hearings to rid the problem asap. But that would take courage, foresight, and leadership.... so not going to happen.
